12/28/2023 EnchantaVaughn Approved QMHPA greeted Ms. Cosby upon arrival to the home setting. Upon arrival to the home setting, Ms. Cosby appeared to be in a foul mood evident by her facial expressions such as a frown on the face while engaging in profanity when communicating with QMHPA. QMHPA instructed Ms. Cosby to refrain from utilizing profanity when communicating with QMHPA. Ms. Cosby apologized for her behavior; however, Ms. Cosby discussed with QMHPA that she was tired of her boyfriend roommate. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by inquiring about Ms. Cosby current thoughts, feelings, and emotions. QMHPA utilized a feelings chart to try and guide Ms. Cosby with verbalizing her feelings beyond the terms of stressed, a skill presented in previous sessions. QMHPA prompted Ms. Cosby to utilize one or more of the coping skills she has been exposed to and processed with her if she has utilized any of them on her own and asked her to gauge how effective she felt any of them were. QMHPA probed Ms. Cosby regarding her eating and sleeping habits and if they had improved since the last session. QMHPA prompted Ms. Cosby to develop a schedule for the week to assist her with accomplishing tasks and not procrastinating or abandoning tasks / appointments altogether. Ms. Cosby appeared to be receptive to QMHPAâs interventions and services that were provided to her during the session. During the session Ms. Cosby admitted that she has not utilized any of the coping skills introduced during previous sessions expressed no desire to improve her feelings of depression and anxiety. She states she is not eating or sleeping well but when QMHPA attempts to redirect her to various tools she could utilize to assist her with those areas she repeatedly said, âI know thatâ then would continue to make statements that suggest she does not desire to move forward today. Ms. Cosby listened as QMHPA educated her regarding organizational tools she could use and how she could take basic steps to complete tasks in a timely manner.
